Job Description

The clinical pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life. Performs all duties in a manner, which promotes team concept and reflects the Sisters of Providence mission and philosophy

Required qualifications:

• PharmD Pharmacy Doctorate or

• Bachelor’s Degree in Pharmacy

• California Pharmacist Registered upon hire

• When eligible and where applicable, obtain a Board of Pharmacy Preceptor license

• Where applicable Basic Life Support-Provider (BLS)

Preferred qualifications:

• Acute care hospital pharmacy experience, 1 year or equivalent experience

• Completion of ASHP-accredited PGY1 residency
